Functional Hypothalamic Amenorrhea

چکیده
Copyright © 2010 Massachusetts Medical Society. A 16-year-old girl presents for evaluation of secondary amenorrhea. Her menarche was at the age of 12 years. Since she started running for exercise and sport at the age of 14 years, her menstrual periods have become lighter and less frequent. Her last menstrual period was 6 months ago. She has lost 2.3 kg (5 lb) over the past 3 months and reports a 2-week history of right foot pain. She typically runs 10 km (6 mi) per day, at least five times per week. On physical examination, her body-mass index (BMI; the weight in kilograms divided by the square of the height in meters) is 19. There is pain on palpation along the fourth and fifth metatarsals; otherwise, the physical examination is normal. How should her case be evaluated and managed?

Background: Patients with a transverse vaginal septum generally present at the age of menarche with complain of cyclic pelvic or abdominal pain due to retention of blood above the obstruction. In the absence of menstruation this women did not have typical hematocolpos. Materials and Methods: A rare case of hypothalamic amenorrhea and transverse vaginal septum were reported. متن کاملNeuroprotection via Reduction in Stress: Altered Menstrual Patterns as a Marker for Stress and Implications for Long-Term Neurologic Health in Women
Individuals under chronic psychological stress can be difficult to identify clinically. There is often no outwardly visible phenotype. Chronic stress of sufficient magnitude not only impacts reproductive function, but also concomitantly elicits a constellation of neuroendocrine changes that may accelerate aging in general and brain aging in particular. Functional hypothalamic amenorrhea, a phen...
متن کاملFunctional hypothalamic amenorrhea and its influence on women’s health
INTRODUCTION Functional hypothalamic amenorrhea (FHA) is one of the most common causes of secondary amenorrhea. There are three types of FHA: weight loss-related, stress-related, and exercise-related amenorrhea. FHA results from the aberrations in pulsatile gonadotropin-releasing hormone (GnRH) secretion, which in turn causes impairment of the gonadotropins (follicle-stimulating hormone and lut...
متن کاملNeuroendocrine and metabolic aspects of functional hypothalamic amenorrhea
Functional disorders of the hypothalamus are the most common reason for amenorrhea. Psychogenic stress, undernutrition and excessive exercise are frequently associated with functional hypothalamic amenorrhea (FHA). It is a nonorganic endocrine disorder characterized by the impairment of gonadotropin-releasing hormone (GnRH) pulsatile secretion. The incidence of FHA ranges from 15% to 55% of the...
متن کاملA genetic basis for functional hypothalamic amenorrhea.
BACKGROUND Functional hypothalamic amenorrhea is a reversible form of gonadotropin-releasing hormone (GnRH) deficiency commonly triggered by stressors such as excessive exercise, nutritional deficits, or psychological distress. Women vary in their susceptibility to inhibition of the reproductive axis by such stressors, but it is unknown whether this variability reflects a genetic predisposition...
متن کاملEstrogen status and the renin angiotensin aldosterone system.
The renin-angiotensin-aldosterone system (RAAS) is integrally involved in multiple cardiovascular physiological processes including arterial blood pressure (BP) regulation. Over activity of the RAAS has been implicated in the pathogenesis of a number of cardiovascular disease entities, including hypertension. Several lines of evidence suggest estrogen favorably modulates the RAAS. Conversely, e...
